Improving Health and Social Care Services for Redbridge residents
As of Monday 4 April 2016, people in Redbridge will receive improved health and social care services with the launch of the new integrated Health and Adult Social Care Service (HASS) between the London Borough of Redbridge and NELFT NHS Foundation Trust.
This unique partnership will see the development of four multi-disciplinary community health teams set up across the Borough based on GP Clusters. The new partnership will have a greater focus on early intervention and prevention to deliver high-quality integrated care and support to people who are over the age of 18 and are:
- vulnerable older people
- adults with a learning disability and/or on the autistic spectrum
- adults with a physical and/or sensory disability
- adults with a mental health issue
By integrating in this way we are seeking to make it easier to access our services, reduce the number of calls and the number of assessments a person has to undergo. We are also seeking to improve the quality of information and advice provided to help people remain as independent as possible.
From 4 April 2016, people will continue to receive the service they are already receiving. If they do need a change of worker any handover arrangements will be worked through with them. As the service develops, people will notice changes, which will include having one worker e.g. social worker, occupational therapist or nurse being your contact for all your care needs.
Each Redbridge HASS cluster team will be made up of social workers, occupational therapists, nurses as well as individual specialist teams e.g. physiotherapy, speech and language, rehabilitation etc. and will be located within the following wards; Fairlop, Cranbrook and Loxford, Wanstead and Woodford and Seven Kings.
There will also be a new online and telephone referral process where a team of specialist Wellbeing Officers and First Contact Officers will provide a co-ordinated approach to offering people advice, information and a wellbeing assessment (if they have this need).
