You may be aware that health and social care services are facing significant pressures. This includes the increased number of people requiring a service and fewer resources with which to provide those services.
The transformation programme ‘For Thurrock in Thurrock’ has been looking at how health and social care partners can find different ways of supporting people to access the right health and social care support.
Local people have already said that it’s difficult to find their way around health and care services, that there’s too much duplication meaning that people have to tell their story several times, and that provision in Thurrock could be improved. People have also shared what they think would help Thurrock residents to be as healthy as possible and have good wellbeing.
The Transformation Programme ‘For Thurrock in Thurrock’ is our response across health and social care partners to what we’ve heard people say.
We think there’s a real opportunity to change the way we do things in Thurrock. We want to develop a set of principles that underpins everything we do and responds to what people have told us.
