District nursing

The district nursing service provides 24 hour care for people aged 16 and over who are housebound (with the exception of leg ulcer clinics) and have an identified nursing need and who live in Havering. The service is provided in the patient’s own home, or in residential care settings, with the exception of leg ulcer clinics, which are held in health centres.

District nurses care for patients with a wide range of nursing needs including the chronically sick and terminally ill. They also provide nursing services to those who are acutely ill and need intensive and technological care or who have specialist care needs.

The Havering community night service, cover the district nursing service out of normal working hours.

Referrals

Referrals will be accepted from GPs and health care professionals via Single Point of Access. Self-referrals are accepted for community nursing teams where the patient meets the service criteria.
