Referrals

Referrals are accepted via email - nelft.haveringreferrrals@nelft.nhs.uk or via the single point of access (SPA).

Criteria: The service is available to Havering residents who are over 60 years old and registered with Havering GPs. The patients must have had three or more of the following:

• a fall in the previous year
• four or more medications
• balance problems
• inability to rise from a chair of knee height
