The children‘s community nursing team provides specialist nursing care for children and young people who have short and long term health needs that cannot be met by mainstream services.

The team visit children and young people in their homes, schools or nurseries to offer specialist nursing support for premature babies, children and young people with cancer, asthma and epilepsy.

The team work closely with other professionals in the hospital and community setting to help prevent children and young people being admitted to hospital and to enable early discharge to a home environment wherever possible.

Referrals

Referrals are accepted from parents/guardians and professionals in health, education or social care. A specialist children's service referral form must be completed, and signed consent from parent/guardian to share information must be provided.
