Self-referral by patients or clients. Referrals can be made through choose and book. Referral form faxed or posted direct to service using the details above, or made by telephone.

Criteria:

This service is available to:

• residents (any age) registered with a GP in Waltham Forest
• people who require screening and/or counselling for a haemoglobinopathy
• people who are living with a major haemoglobinopathy (sickle cell disorder of thalassemia)
• social, education and health professionals or carers who require advice and education on caring or providing a service
