Urgent Community Response Team (UCRT Thurrock)
The Urgent Community Response Team is a joint initiative across Mid & South Essex consisting of teams from Provide, NELFT and EPUT. You may already know the service in Thurrock as RRAS. The service responds to the crisis needs of patients in their own homes, including nursing and residential care homes, to avoid admission to hospital or the need of new 24 hour care.
The service has access to a range of specialists including: senior nurse assessors, independent non-medical prescribers, generic support workers and social workers to support patients to remain in their usual place of residence during an acute episode of illness or urgent social care crisis.
The UCRT service also has access to the secondary care consultant helpline as well as therapy services for adults that are experiencing a sudden decline in mobility or function or have experienced a fall. The UCRT are able to provide equipment and home adaptations to optimise functional ability and safety at home.
