Community learning disability team (CLDT)
The Waltham Forest community learning disability team (CLDT) provides assessments to determine the best way to support adults with learning disabilities and their carers to be as independent as possible and to maintain their health and wellbeing. The CLDT is an integrated team of social care and health staff. This team is made up of: community learning disability nurses, psychiatrists, psychologists, occupational therapists, speech and language therapists, physiotherapists and social workers. The service offers therapy clinics and drop-in sessions.
The social work team provides advice and carries out assessments under the Care Act. They also help plan and arrange care and support for adults with learning disabilities and their carers. After the assessment they can help people access the services they need such as day care, respite care, health services, education, employment support, occupational therapy, physiotherapy, and speech therapy.

NELFT's Waltham Forest Community learning disability team (CLDT) are running a series of online videos for patients and service users during lockdown. These videos include crafts, baking, mindfulness and even easy read news videos.

Referrals will be accepted through GP, carer, or telephone call to the team. Self-referrals will also be accepted.
