Community learning disability team (CLDT)

The Waltham Forest community learning disability team (CLDT) provides assessments to determine the best way to support adults with learning disabilities and their carers to be as independent as possible and to maintain their health and wellbeing. The CLDT is an integrated team of social care and health staff. This team is made up of: community learning disability nurses, psychiatrists, psychologists, occupational therapists, speech and language therapists, physiotherapists and social workers. The service offers therapy clinics and drop-in sessions.

The social work team provides advice and carries out assessments under the Care Act. They also help plan and arrange care and support for adults with learning disabilities and their carers. After the assessment they can help people access the services they need such as day care, respite care, health services, education, employment support, occupational therapy, physiotherapy, and speech therapy.

Intensive Support Team for people with a Learning Disability and Autism

The Intensive Support Team for people with a learning disability and autism aims to enhance the current function of the Adult Community Learning Disability Teams (CLDTs) across NELFT.

The service provides intensive support for those with more complex needs, offers (through consultancy and preventative and proactive working) rapid and intensive support to ensure, as far as is reasonably practicable, that a user of service remains in their own home or environment and relies on joint working with the CLDTs.  

The Team consists of: A Consultant Psychiatrist, Speech and Language Therapist, Behaviour Specialist, Occupational Therapist, Senior Nursing staff, and Support, Time, and Recovery workers.  

Within normal working hours (Monday to Friday 9am to 5pm) your first point of contact is the Waltham Forest Community Learning Disability Team. The Intensive Support Team is a 24-hour service and calls are directed to the out of hours team for support required after 9PM, during weekends and bank holidays. Referrals

Referrals will be accepted through GP, carer, or telephone call to the team. Self-referrals will also be accepted.
