Referrals

Referrals are accepted on the ‘continence referral form’ (see attached) e-mailed to:
nem-tr.continenceserviceredbridge@nhs.net

Criteria: Patients over the age of 18 with continence needs requiring specialist nursing advice and intervention; patients who are registered with a Redbridge GP.
Exclusions: Patients currently on a District Nursing or Community Matron caseload requiring basic continence assessment
