Redbridge HASS community Therapies

We are an integrated health and social services (HASS) team, working within the London borough of Redbridge HASS locality clusters.  As a team we provide the therapy services across all four of the HASS clusters. We work in an integrated manner with all members of the HASS MDT team including community matrons, district nurses and social workers.

We have two teams in Redbridge HASS community Therapies, these are:

The Stroke and Neuro Team   

Our Stroke and neuro team consists of physiotherapists, occupational therapists, rehab support workers (RSWs), a stroke clinical psychologist and a motor neurone disease (MND) coordinator.  All our team have specialist knowledge, expertise and experience working with adults who have had a stroke or have a neurological diagnosis including conditions such as, MND Multiple Sclerosis, Parkinson’s and traumatic brain injuries.  

The General Rehab Team

Our General rehab team consists of physiotherapists, occupational therapists, rehab support workers (RSWs), a stroke clinical psychologist and a motor neurone disease (MND) coordinator.  All our team have specialist knowledge, expertise and experience working with housebound adults, with a wide range of conditions including, elective or traumatic orthopaedic conditions (such as hip and knee replacements or orthopaedic surgeries).  Musculoskeletal conditions where the person is unable to attend an outpatient clinic and general deconditioning and deterioration at home resulting in a loss of independence with activities of daily living.

Our aims

Our aim is to provide a personalised goal orientated therapy programme to people within their own homes.

We provide specialist therapy assessments for functional tasks and activities of daily living, including washing and dressing, meal preparation and upper limb rehab.  We provide falls, balance, stair, and transfer and mobility assessments.

The aims of our assessments and therapy treatment programmes are to enable people to remain as independent as possible within their own homes and reduce their need for care support.

Referrals

We accept referrals from any professional members of staff including GPs, acute hospital staff , social services, HASS professionals and other AHPs and nurses.  We do not accept self–referrals into our services.

We use one referral form for both our stroke and neuro team and our general rehab team, which can be requested from our admin team using the email address listed.

We triage all our referrals on a daily basis and will inform the referrer if we are unable to accept the referral made to us if we require further information on the referral in order for us to triage and prioritise effectively.

The teams waiting list times are variable depending on the number of referrals being received.  For an up to date waiting list time please contact our admin team.
