New referral process for Waltham Forest adult community health services.

As of Monday 5 September 2016, residents of Waltham Forest who are over the age of 18 will be able to access the new model of care for Adult Community Health Service.

Improving community healthcare for Waltham Forest residents

Adult Community Health Services will operate as Unplanned (admission avoidance and urgent care/Rapid Response) and planned care divisions, supporting the integrated care delivery pathway developed by the CCG.

The new model will improve patient access and help to reduce delays; ensuring patients receive care within a clinically appropriate timeframe, location, and by the most suitable clinical service function.

The services delivered as part of planned care will be aligned around three GP localities as below.

District nursing and the Integrated Care Management teams will operate as Integrated Community Teams (ICT). The ICTs will make it easier to access the services, reduce the number of calls and the number of assessments a patient has to undergo. Professionals, patients, carers and families will be able to access the service through a Single Point of Access (SPA).

The new ICTs will support early intervention and self-management to enable people to manage their own conditions and prevent development of ill health with the aim of people being able to remain as well as possible for as long as possible within their own homes and communities. This means that more care will be delivered out of hospital and in people’s homes as well as in the wider community. 

Each of the three ICT clusters will be made up of community nurses, health care assistants, physiotherapists, occupational therapists, as well as individual specialist teams e.g. therapy rehabilitation services, palliative/end of life care, tissue viability, podiatry, continence, nutrition and dietetics, Parkinson’s, multiple sclerosis and haemoglobinopathy services.

There will be a new online and telephone referral process as part of the new internal SPA team. The SPA will provide a coordinated approach for the referrals received, and when relevant, signpost people to the appropriate services that are external to NELFT.

The new service was officially commissioned by the CCG from 4 April 2016.
