COMPASS brings together existing palliative and end-of-life care teams in Waltham Forest, including the Barts Community Specialist Palliative Care (CSPC) team and the NELFT Enhanced Palliative Integrated Care Service (EPIC).

By working as one coordinated service, we can provide a more seamless, joined-up experience for patients, families and carers.

Operating Hours 

Monday to Friday, 9am – 5pm

Criteria

The service supports adults (18+) with a GP registered in Waltham Forest who are in the last year of life or living with a progressive, life-limiting illness and have complex palliative care needs, with symptoms that are difficult to manage, as well as emotional or social distress affecting them or the people who care for them.

Referral information

Referrals can be made by GPs, healthcare professionals, care home staff, or by patients and families themselves by emailing: wfadultchsreferrals@nelft.nhs.uk

Referrals by healthcare professionals MUST CONTINUE on a Pan London PallI8 form via the SPA: wfadultchsreferrals@nelft.nhs.uk   

Referrals with inadequate information or without the patient’s consent will be rejected and the referral will not be accepted.
