Francis Fernando (Associate Director of Nursing and Quality  - Waltham Forest) has shared his fishbone diagram which he created as part of his Harvard Business School on-line Organisational Leadership Certificate. This is currently being further developed by the Waltham Forest team looking at a workforce case and effect elements, as well as data being gathered, to build a picture of potential quality improvement approaches in the near future.
