Intermediate Care Therapy

The CHSCS physiotherapy/occupational therapy team provides short-term rehabilitation in a patient’s own home to improve their level of independence and safety. The patient is encouraged to resume daily life activities through rehabilitation in order to improve their confidence and maintain their independence.

The team consists of occupational therapy and physiotherapy services. The occupational therapy service aims to increase independence and safety with personal care tasks and other functional tasks. The team assess and advise patients regarding aids and adaptations such as stair-rails, grab-rails and other equipment as required.

The Physiotherapy service aims to improve patients’ strength, mobility, balance, exercise tolerance and confidence by providing exercise programs and walking aids as necessary.
The services support patients who require:

  • short-term (up to six weeks) community therapy and rehabilitation in their own homes
  • prevention of hospital admissions
  • early discharge home following acute admission conditions where patients experience reduced mobility and functional ability

The Intermediate Care Therapy Team is part of the Community Health and Social Care Service (CHSCS).
