Community rehabilitation service

The community rehabilitation service provides assessment and rehabilitation to patients aged 16 and over who have been diagnosed with a neurological condition.

Neurological conditions include:

  • Parkinson's disease
  • Multiple sclerosis
  • Motor neurone disease
  • Stroke

Therapists see patients within their own home or in the local community and there is access to a gym for outpatient sessions / group work as appropriate to the patient's needs. The service consists of physiotherapists, occupational therapists, speech and language therapists, a dietician, rehabilitation and profession specific assistants, and administrative support. Therapy is patient centred, goal orientated and therapists work with their patients to enable them to develop skills or strategies to be able to participate in meaningful activities and/or manage their own symptoms.
