Referrals can be made by telephone, but must be supported by a Community Children’s Nursing Referral Form. Please call above number for referral form

Referral Criteria         

Resident in South Essex area. 
Patients must be 0-19 years of age.
The child or young person must have a cancer diagnosis or life – limiting / threatening condition.
