Referrals

Referrals will be accepted form GPs and health and social care professionals. Self referrals will also be accepted from patients and clients.

GP Referrals (EMIS Practices)

If you are a GP using EMIS, please submit all new routine referrals to this service via Elemental. This is now the standard referral route into NELFT for EMIS practices and replaces the previous email-based process.

 

External Referrals

If you are referring a patient to this service from outside NELFT, including GPs using SystmOne, please submit all new referrals using our online external referral form: Referral form
 
Urgent referrals

If you need to make an urgent referral, please continue to use the existing urgent referral pathways and contact the service directly where required. The online referral form should not be used for urgent referrals.
