Referrals

Referrals will be accepted by faxing or posting the referral form the contact details listed on Patient information. Self-referrals will also be accepted by telephone.

Criteria:

BMI >25
CVD Risk > 10%
Smokers wanting to quit smoking
Pregnant smokers
