Referrals

Referrals are accepted from GP practices, accident and emergency departments, walk in centres, social services, children centres and other NHS trusts – or simple self-referral from clients directly. Referrers can contact the team via our duty desk, where the named health visitor will be identified if the child is already known to our service, or a named health visitor will be allocated if they are not.

If the child is of school age – the school nurse will be identified by confirming the child’s school attended.

This service is available to babies and children up to 19 years old, living in the London Borough of Barking and Dagenham.
