The integrated care team ensures patients over the age of 18 with complex health and social care needs receive the right care, in the right place and at the right time. The team works with health and social care providers to co-ordinate and offer multi-disciplinary quality care to vulnerable adults. This integrated care team is divided into six clusters across Barking and Dagenham.

Referrals

To access this service patients need to be registered with a Barking and Dagenham GP. Referrals are made by GPs and healthcare professionals. Self referrals can be made by patients and clients.

For urgent matters, out of hours service transfers to Community Treatment Team (CTT) ( Mon - Sun  8am - 10 pm)
