Neuroscience specialist nursing service
The neuroscience specialist nursing service supports people over the age of 18 living with neurological conditions. The service helps patients avoid unnecessary hospital admissions and outpatient appointments. Patients are seen either at nurse-led clinics or in their home environment if required.
The neurology nurse specialist service provides:
- medication review, advice and information
- advice and information on condition
- opportunity identify concerns with patients and their carers and discuss ways of managing them
- liaison between neurologist, GP and geriatrician regarding all aspects of care
- liaison and referral to other healthcare professionals
Clinics are also held at Five Elms Medical Centre.

The above services accept referrals from health care professionals as well as self referrals from patients or their carers.
