Referrals

Referrals will be accepted by using the  MSE Heart Failure Referral Form (002).docx [docx] 5MB. Patients can also self-refer.

Criteria: Confirmed diagnosis of heart failure via echocardiogram.

Test results required at point of referral: BNP (if available) and/or echocardiogram.
