Referrals

GP Referrals (EMIS Practices)

If you are a GP using EMIS, please submit all new routine referrals to this service via Elemental. This is now the standard referral route into NELFT for EMIS practices and replaces the previous email-based process.

Urgent referrals: If you need to make an urgent referral, please continue to use the existing urgent referral pathways and contact the service directly where required. Elemental should not be used for urgent referrals.

This change applies to GP practices using EMIS. Alternative referral routes remain available for practices using other clinical systems.

Other referrals will be accepted via phone call or letter direct to the older adults access team, e-referral is also available for GPs. All new referrals need to be made via the GP to exclude/treat physical causes for presenting problems.

Criteria: Any person aged 18 and over with dementia or suspected of having dementia Resident in Havering

Exclusions: None

Test results required at referral: If suspected dementia, please complete routine blood tests and any other tests that are clinically relevant to exclude other causes of cognitive impairment (e.g. MSU if indicated).
