Referrals

GP Referrals (EMIS Practices)

If you are a GP using EMIS, please submit all new routine referrals to this service via Elemental. This is now the standard referral route into NELFT for EMIS practices and replaces the previous email-based process.

Urgent referrals: If you need to make an urgent referral, please continue to use the existing urgent referral pathways and contact the service directly where required. Elemental should not be used for urgent referrals.

This change applies to GP practices using EMIS. Alternative referral routes remain available for practices using other clinical systems

Other referrals will be accepted through single point of access (SPA) by email nelft.haveringreferrals@nelft.nhs.uk from consultants, GP practices and health care professionals (band 5 and above). Partially completed referrals will be rejected. Referrals are accepted for patients with a Havering GP.  Referrals must include:

• Current weight
• 3-6 month weight history (adults) or growth measurements (paediatrics),
• Height/length
• Malnutrition Universal Screening Tool score (adults) ‘MUST’ Calculator | BAPEN. Please see below for guidelines.
• Medical history
• Results from investigations (particularly for allergies, intolerances, IBS, diabetes & CVD).
• Adult enteral feeding discharge documents where relevant.

Adult Enteral Feeding

• Registrations & requests for ancillaries must be completed by referring team in line with this standard operating procedure. Please email the team to request a copy.
• Please note that ancillary equipment is supplied in line with local SOP for community residing patients.  

Adult Malnutrition

There are local guidelines from North East London ICB titled “Identification, Treatment and Management of Malnutrition in Adults” Medicines Guidelines – Nutrition & Blood – North East London. This includes resources for patients and care homes. Please ensure referrals are in line with above when requesting advice on supplements.

Weight Management

The NHS Digital Weight Management Programme supports adults living with obesity who also have a diagnosis of diabetes, hypertension or both, to manage their weight and improve their health. Please refer patients to: NHS England » The NHS Digital Weight Management Programme
