Referrals

GP Referrals (EMIS Practices)

If you are a GP using EMIS, please submit all new routine referrals to this service via Elemental. This is now the standard referral route into NELFT for EMIS practices and replaces the previous email-based process.

Urgent referrals: If you need to make an urgent referral, please continue to use the existing urgent referral pathways and contact the service directly where required. Elemental should not be used for urgent referrals.

This change applies to GP practices using EMIS. Alternative referral routes remain available for practices using other clinical systems.

Other referrals can be emailed or posted using the single referral form through single point of access (SPA).

Criteria:

• Children and young people between 0-16 years old, aged between 17 -19 years old with an EHC Plan, living in or registered with a GP in Redbridge
• Children identified as having reduced gross motor skills due to a neurological condition
• Children between 0-12 years old with musculoskeletal problems
• Babies born with congenital conditions
• Children with conditions including: developmental delay; co-ordination difficulties; genetic conditions affecting movement, chronic pain that would benefit from physiotherapy modalities
