Referrals

Referrals will be accepted from health and social care professionals.

GP Referrals (EMIS Practices)

If you are a GP using EMIS, please submit all new routine referrals to this service via Elemental. This is now the standard referral route into NELFT for EMIS practices and replaces the previous email-based process.

Urgent referrals: If you need to make an urgent referral, please continue to use the existing urgent referral pathways and contact the service directly where required. Elemental should not be used for urgent referrals.

This change applies to GP practices using EMIS. Alternative referral routes remain available for practices using other clinical systems.

Other referrals can be made through the single point of access using the details above. Self referrals by patients and carers can also be made.

Criteria: People aged 16 and over registered with a GP in Waltham Forest. People who have a specific nursing need that requires the expertise of a nurse.

Referrals will only be accepted if the patient is housebound. For example, if they are only able to leave home by ambulance, or there is a nursing need that makes a home visit more appropriate, such as palliative care.
