
Congratulations to Jayshree Sharma, Lead Pharmacist – Frailty and Older People, who has been recognised nationally with an Unsung Hero award at this years Clinical Pharmacy Congress Awards.
These awards honour standout individuals and teams across the pharmacy profession - those who are driving innovation, improving patient outcomes, and making a lasting difference.
Jayshree was nominated by colleagues from Mid and South Essex ICS and NHS Mid Essex CCG, who praised her leadership in older adult care and frailty management:
"Jayshree played a key role in a successful nurse-pharmacist pilot that focused on holistic, patient-centred care - deprescribing Falls Risk Increasing Drugs (FRIDs), reducing medication costs, and potentially lowering hospital admission rates, working closely with nursing colleagues in Thurrock Community Frailty team. She also delivered pharmacist training, coached nurses on medication safety, and led Falls Week initiatives - designing patient education sessions on rehabilitation wards and facilitating open discussions with patients and healthcare staff around falls risks and medication-related concerns. Her contributions to memory services in NELFT’s London services, workforce education, and advocacy underscore her leadership and dedication to patient safety."
Jayshree has also been selected to be on the Dementia Clinical Leadership Group for NHS England – London.

“As Chief Pharmacist I am extremely proud of the innovative work that Jayshree has done both as an individual specialist pharmacist and as part of these two MDTs. These are great examples of achieving improved patient outcomes when professionals work together to ensure the right skill mix in teams.”
