Referrals

Referrals from GPs and services outside of NELFT should use the Barking and Dagenham Memory Service referral form.

Referrals from services within NELFT should follow the new internal referral process. For our team, this requires a clinician-to-clinician discussion.

Referrals can be agreed through either a scheduled consultation or by contacting the team via the generic email address: BDmemoryservice@nelft.nhs.uk
