Intermediate care and community stroke service (CHSCS)
Intermediate care provides short term (up to 6 weeks), goal focussed interventions to promote and facilitate independence following a loss of ability due to illness at home or in hospital.
Therapy staff carry out assessments and act as keyworkers to coordinate care and rehabilitation with a variety of essential and valued daily activities. We aim to have people regain independence. However, in some cases longer term support is necessary; this is arranged through health and social care colleagues such as community matrons, specialist nurses and social workers.
