
The Child Health Hub is a new service in Havering that brings various BHRUT, NELFT & Primary care doctors, nurses, school health staff, social workers, and other health care professionals together as one team.
Instead of working separately, they share knowledge and work side by side to give your family the right help, at the right time.
Every child deserves the best start in life. The Hub makes sure care is:
Easier access to support – less running around between services.
Joined-up care – professionals talk to each other, so you don’t need to repeat your story.
Faster connections – if extra help is needed, the Hub links you quickly to the right service.
Stronger safety net – children at risk of difficulties are supported earlier.
Family-centred care – your child’s health and wellbeing are looked at in the context of your whole family.
If you are referred this may be for a discussion or a clinic. If you are chosen for a discussion, a group of professionals will discuss the case and decide a plan for your child, your GP should then advise you of the plan. If you are chosen for a clinic, you will be informed of the date, time and location of the clinic, you would be expected to attend with your child to speak with a few professionals to decide a plan for your child.
GP’s can refer patients who are 0-18 years or up to 25 years with SEND via JOY app, which is available for GP’s to access via their practice systems.
